A US airstrike hit a hospital run by Medicines Sans Frontier (Doctors without borders) in Kundunz, Afghanistan last year and recorded 42 casualties.

The United States has since apologized for the airstrike and a US Army commander termed it a “tragic but avoidable accident caused primarily by human error.”
Over a dozen military officers have been punished in connection with the attack although the nature of their punishment has not been made public. The airstrikes were meant to have targeted a Taliban command center, some 400 yards from the MSF run hospital.
